Foundational

Assessment

The Foundational Assessment is just that, the foundation of your care journey - and is designed to move beyond symptom management to help you understand the root cause, the "why" behind your symptoms, and what can be done to correct them at the root cause. The Foundational Assessment is a one-time, 3-visit cluster of appointments that serves as the foundation and springboard for your care journey. This assessment can be used as a standalone evaluation for optimal wellness or as the required entry point before joining the ongoing Next Chapter Partnership Membership.

What Is Offered:

Clinical Components: Throughout the assessment, you will receive a comprehensive evaluation that includes:

- A comprehensive history, symptom review, and goal assessment.

- A physical examination.

- An InBody 580 Body Composition Analysis.

- A detailed review of Advanced Biomarkers, which look at metabolic markers, nutrient status, an expanded thyroid assessment, inflammatory markers, lipids, and hormones.

The Process (The 3-Visit Cluster)

1. Initial Visit (75 to 90 minutes, in-person). This is a deep-dive clinical assessment with Dr. Marshall. During this comprehensive visit, you will:

- Perform the InBody body composition assessment.

- Review your current symptoms, health history, and goals.

- Undergo a physical exam.

- Review your advanced biomarker lab results.

- Discuss the possible underlying patterns contributing to your health concerns and begin outlining the next steps to build your personalized care roadmap.

2. Brief Care Plan Review (Within a week of the Initial Visit) This is a dedicated follow-up session to thoroughly review your personalized roadmap. During this visit, you will:

- Review Dr. Marshall's overall impression of what is driving your symptoms at a root level.

- Receive foundational recommendations covering nutrition, sleep, exercise/movement, stress management, relationships, and supplements (including a 15% discount via Fullscript, if needed).

- Discuss recommendations for further advanced testing and receive educational handouts.

- Learn how to access your resources, get your questions answered, and explore ongoing membership options.

3. Implementation Visit (2-3 weeks after the Care Plan Review): This is a short follow-up visit focused on navigating your new plan. This is conducted by our Nurse Health Coach and is designed to:

- Review how the implementation of your care plan is going.

- Discuss your wins and overcome any challenges or obstacles.

- Answer any remaining questions you may have.

- Revisit membership options to ensure you feel clear, confident, and supported if you choose to transition seamlessly into ongoing care.

Price, Payments, and Lab Details

The Investment: The total cost for the Foundational Assessment is $595.

Payment Options: You can use HSA/FSA funds, and upon request, you will be provided with a superbill to submit for possible out-of-network insurance reimbursement.

Medicare Notice: Dr. Marshall is a Medicare opt-out provider, meaning these services are not reimbursable by Medicare.

*Lab Costs: The cost of lab work is not included in the assessment fee. Routine labs can be billed through your insurance (though it is not recommended), or you can utilize the practice's negotiated cash pricing through Quest. For added convenience, SAS employees and patients have the option to have their blood drawn directly at the SAS campus.

What is the Next Chapter Partnership Membership? The Next Chapter Partnership Membership is an ongoing care plan, referred to as the "Partnership Model," designed exclusively for patients who have already completed the Foundational Assessment. It is meant for individuals who are ready to take a deeper dive into addressing chronic dysfunction by uncovering the root causes of their symptoms, diseases, or lab findings. This model focuses on utilizing advanced tests and personalized protocols to provide the ongoing support and communication necessary to achieve profound, lasting healing.

Price and Payment Details:

The Investment: The membership is a 12-month commitment with a recurring monthly charge of $249 (totaling $2,988 per year).

Additional Visits: If you need more time with the doctor beyond the included visits, additional appointments are billed separately at $299 per visit.

Payment Options: You may potentially use HSA/FSA funds to pay for the membership, though it is recommended to check with your plan administrator first.

Medicare Notice: Dr. Marshall is a Medicare opt-out provider, which means these membership services are not reimbursable by Medicare.

**Exclusions: Because they are highly personalized to each patient, the costs of any advanced lab tests and supplements are not included in the membership fee and are billed separately.

Throughout your 12-month membership journey, you will be provided with a structured roadmap of support that includes:

Comprehensive Doctor Visits: Five 60-minute clinical visits directly with Dr. Marshall.

Implementation Visits: Five 30-minute telehealth visits with our Nurse Health Coach that focus on behavior and lifestyle interventions.

Advanced Diagnostic Testing: The ordering, interpretation, and treatment implementation of advanced diagnostics not typically used in conventional medicine. This deep dive into your health can include advanced stool microbiome analysis, adrenal profiles, breath tests, and organic acid analysis.

Continuous Support: Ongoing portal messaging directly with the doctor and the nurse health coach, ensuring continuous communication between visits.

Supplement Discount: A 15% discount on all supplements ordered through Next Chapter’s online dispensary (Fullscript).

InBody Body Composition Analysis

Rather than relying on a misleading bathroom scale, true health requires looking beyond weight alone. To better understand your body's interconnected systems, we need more than a single number on the scale. The InBody 580 is a medical-grade, non-invasive body composition assessment that, in just 30 seconds, provides a detailed picture of what your body is made of, including muscle, body fat, and hydration. This allows you to track meaningful changes in your health, not just how much you weigh.

**This InBody Analysis is included with the Foundational Assessment. However, it is offered as a stand-alone service for those only interested in body composition analysis.

What is Offered?

An InBody analysis is a non-invasive screening tool that uses bioelectrical impedance analysis (BIA) to estimate body composition measurements, including but not limited to body fat percentage, skeletal muscle mass, total body water, and related metrics..These measurements may be useful for:

- Tracking changes in your metabolism and body composition over time

- Monitoring muscle mass to help maintain strength and healthy aging

- Watching changes in inflammation, hydration, and fluid balance

- Helping measure progress and stay accountable

The Process

To get started with the InBody Assessment, you will follow a simple, streamlined process:

2. Complete an intake form through the practice's Electronic Health Record system.

3. Schedule your appointment to come in for your measurement.

Price Options

The assessment is offered in three different pricing tiers depending on your needs:

$75 (The Deep Dive): This covers your initial body composition analysis along with a 30-minute physician consultation.

$45 (The Progress Check): This includes a follow-up analysis and a 30-minute physician consultation to continuously monitor your progress.

$25 (Measurement Only): A quick "stop-in" follow-up appointment strictly to take your measurements and track your data, without a physician consultation.
